Change Banking Details Form 47: Financial Institution Account Details - Senators and Members

Electorate Staff Allowance Allocation Form 119: Electorate Staff Allowance Allocation  By 1 July each year or within 2 weeks of a trigger event occurring. Scan and Email to: MOPSPay&Conditions@finance.gov.au; or
Post or hand deliver to M&PS.
 
Invoice Certification - Office Requisites and Stationery Form 155: Invoice Certification - Office Requisites and Stationery   Scan and Email or Post to your relevant State Office. Tax Invoice MUST be attached to the claim.
Where reimbursement is required, proof of payment must also be attached.
Invoice Certification - PPV and Other Form 146: Invoice Certification Reimbursement – PPV and Other   Scan and Email, Fax or Post to your relevant State Office. Tax Invoice MUST be attached to the claim.
Where reimbursement is required, proof of payment must also be attached.
Invoice Certification - Publications Form 145: Invoice Certification - Publications   Scan and Email, Fax or Post to your relevant State Office. Tax Invoice MUST be attached to the claim.
Where reimbursement is required, proof of payment must also be attached.

Residential Telephone Services Form 16: Nomination of Telephone Services in a Private Residence   Scan and Email, Fax or Post to your relevant State Office.  
Termination of Employment Form 107: Termination of Employment  Must be completed each time an Employee employed under the MOP(S) Act finishes employment either through resignation or termination. Scan and Email to: MOPSPay&Conditions@finance.gov.au; or
Post or hand deliver to M&PS.
Copy of termination or resignation letter should be attached.
