MOP(S) Act employment agreement forms go digital

MOP(S) Act employment agreement forms can now be accessed, completed and submitted online, on the MaPS website.  Online forms include:

  • Casual employment agreement (Form 117)
  • Ongoing employment agreement (Form 109)
  • Non-ongoing employment agreement  (Form 106)
  • Variations to ongoing and non-ongoing employment (Form 102).

There are plenty of benefits to submitting employment agreements online including increased efficiency (no printing and scanning!) and a streamlined approval process.

What has changed and what will stay the same?

The information required to submit your employment agreements has not changed.  The process to complete, submit and approve employment agreements has changed. And, your form’s progress can now be saved. After filling the form, hit Save. When you return to your browser*, you won’t need to enter details again, provided they haven’t changed.


When commencing employment, you will now complete and submit your employment agreement and/or variation/s, along with supporting documents, online. Once submitted, a copy of the form submission will be emailed to you for your records.

Employers/authorised officers

When the commencing employee submits the completed form online, you will receive an email with instructions. Follow the instructions and forward to MaPS for processing.

Visit the employment agreements landing page and choose the employment agreement applicable to your situation. Ensure you let your staff know that employment agreements should now be completed online and look out for more digitised forms over the coming months.
