Who should complete this form
MOP(S) Act employees wishing to access Parental Leave (including Maternity Leave) as primary or secondary caregivers.

Submission requirements
- Applications for parental leave should be submitted to Ministerial and Parliamentary Services as early as possible and no later than six weeks prior to the expected date of birth or placement of child.
- Pregnant employees must include a written statement from a medical practitioner or midwife confirming the expected date of birth.
- Parental leave for a pregnant employee commences six weeks before the expected date of birth of the child and continues until six weeks after the date of confinement.
- Where a pregnant employee wishes to:
- defer the commencement of leave; and/or
- return to work earlier than six weeks after the birth of the child,
the application should include a certificate from a registered medical practitioner (e.g. general practitioner, obstetrician) confirming that the employee is fit to work during this period. Please note: a midwife is not considered a medical practitioner for this purpose.
- Applications for secondary caregivers must include evidence that the child is born to them (e.g. birth certificate or hospital documentation) or has been placed into their care.
