When to use this form

This form can be completed by the Parliamentarian, Authorised Officer or the nominee.

Workplace type
Only WHS Site Officers can apply for this workplace type.

Employment status
Casual and voluntary staff do not meet the criteria to undertake WHS roles.
Part time staff must work a minimum of 15 hours per week to be eligible to undertake a WHS role.
