Mode of submission

  • Complete the form and submit.
  • The Employer or Authorised Officer will receive an email to be forwarded to MaPS for processing.
  • You will receive a copy for your records.

Submission requirements

  • Please note that reasonable relocation costs may be payable to ongoing staff who relocate permanently.
  • Additional information to help MOP(S) Act Employees who are relocating can be found on form 74 - relocation checklist.
  • Application should be submitted prior to relocation as approval may be required for some costs.
